An LTD may have a minimum of one director. (For third and subsequent directors, please use the "Additional Director" section later in this form).

The sole director of an LTD cannot act as company secretary.

As a regulated trust or company service provider ("TCSP"), we are required to carry out client identification procedures. Please arrange to let us have the following for each person who will be a director of the new company or any person who will be the owner of, or the holder of an option to own, 25% or more of the equity or voting capital of the Company:

1. Clear certified copy of a fixed household utility bill (electricity, gas, broadband), bank statement or correspondence from an Irish Government Department or the Revenue Commissioners, dated within the past 3 months, and addressed to the individual at the home address listed on this order form.

A mobile phone, car insurance or health insurance bill will not be accepted.

2. Clear colour certified copy of their current passport, driving licence, public services card or national identity card.

Certification can be provided by a practising accountant or solictor or a member of a national police force.
